Disability Healthcare Network
Children and adults who are medically frail are supported in special accommodation by Nursing and other Care staff.
Where
The Cram Foundation has five group homes located in the Illawarra area for a total of 23 residents. Each Group Home has between 4-5 residents.
Support available
Registered nurses and non-nursing staff are trained to meet the specific needs of individual residents and provide 24 hour support. Services include personal care, housekeeping, learning, social support, access to the community and assistance with budget management. Each home has its own wheelchair accessible transport.
The Foundation’s Clinical Nurse Consultant assists staff in developing Health Care Plans and maintaining good health care.
Service Planning
Three months after entry an Individual Service Plan is prepared with the resident and their family. This Plan forms the basis of service delivery to meet the resident’s needs and choices of life in the group home. Plans are reviewed with the resident at least each three months and revised annually.
Service monitoring
• The Foundation has management guidelines for monitoring Group Home operational services three times each year. Reports are provided to the Board and the relevant Group Home staff
• The Community Visitor visits each group home once each six months and provides a report to the NSW Minister for Disability Services. For more information on the Community Visitor Programme contact www.ombo.nsw.gov.au
• The Foundation is subject to formal service monitoring by the Funding agency- the Department of Ageing, Disability and Home Care.

Attendant Care – High Needs Pool
Services are provided for persons who have complex care needs, are living in their own home, and require support to help them live independently.
Where
Services are available across the Southern Highlands area.
Support offered
Support from suitably trained and skilled staff of up to 35 hours per week can be provided for assistance with personal hygiene, health care tasks, and daily domestic and other living activities.
Service Planning
Services will be provided in accordance with the client’s own plan to manage their support. Assistance to develop this Plan can be provided on request.
Service Monitoring
• Service users are invited to respond to periodic service surveys from the Foundation
• The Foundation is subject to formal service monitoring by the Funding agency- the Department of Ageing, Disability and Home Care.

In-Reach Support
Case management and activity support for young people living in residential aged care (Nursing Homes) to assist them to link in to and be part of their community.
Where
Support is provided to persons living in the Illawarra area.
Support offered
Support from suitably trained and skilled staff can be provided to assist in renewing or commencing learning and social activities in the community.
Service Planning
Three months after entry an Individual Service Plan is prepared with the client. This Plan forms the basis of service delivery to meet the client’s needs and choices. Plans are reviewed with the client at least each three months and revised annually. Clients may choose to have copies of all plans and reviews.

• Service users are invited to respond to periodic service surveys from the Foundation
• The Foundation is subject to formal service monitoring by the Funding agency- The Department of Ageing, Disability and Home Care.

Lifetime Care and Support
Services for children and adults who have been catastrophically injured in a motor vehicle accident. Support is provided to enable them to return to and live in their own home.

Support is provided to persons living in the Illawarra area.
Support offered
Support from suitably trained and skilled staff can be provided for assistance with personal hygiene, health care tasks, and daily domestic and other living activities.
Service Planning
During the preparation to leave hospital, an Individual Service Plan is prepared. The Foundation can assist in the development of the Plan at the person’s request. This Plan forms the basis of service delivery to meet the client’s needs and choices about support. Plans are reviewed with the client at least each three months and revised annually.

• Service users are invited to respond to periodic service surveys from the Foundation
• The Foundation is subject to formal service monitoring by the Funding agency- the Department of Ageing, Disability and Home Care.

Disability Nursing Consultancy
The Foundation can assist other Organisations in health care assessment, implementing Health Care Plans, and monitoring of health care services. Service Systems review and support is also available. All consultancy services are on a fee for service basis.
